Medicaid Service Coordination

Plan of Care Support Services (PCSS)

PCSS is available to people of all ages. To obtain PCSS you must:

Be considered eligible through OPWDD. Some qualifying disabilities which occur before age 22 are mental retardation, cerebral palsy, autism spectrum disorders, neurological impairments, seizure disorders, and traumatic brain injury
Be any age
Be available for two contacts per year to review the Individualized Service Plan (ISP) and monitor HCBS services.
Be enrolled in an HCBS waiver service

Family Support Case Management

A case manager can be available to assist individuals in enrolling in programs and services. Both short and long term follow along is available. In order to recieve Family Support Case Management:

A person must have a documented diagnosis of a developmental disability
A person's needs for assistance must be short or long term
Medicaid enrollment is not required

NYS Elks Home Service Program

A case manager can be available to assist individuals in short-term linkage and referral to services available in their communities. To qualify for Home Service Director assistance, a person would have similar needs as someone with a developmental disability.
